This is a fully remote position, open to applicants in Ohio.
• Design, evaluate, and manage care coordination activities tailored to member requirements
• Create or assist in crafting individualized care/service plans
• Inform members and their families/caregivers about available services and benefit options
• Assess member needs, obstacles to care, existing resources, and propose or facilitate care strategies
• Identify necessary providers, specialists, and community resources for comprehensive care
• Offer psychosocial and resource assistance for employment, education, housing, food, participant direction, independent living, justice, and foster care needs
• Connect members, families, and caregivers with care provider teams to guarantee timely access to services
• Track progress towards care plan objectives and monitor any changes in member status
• Collaborate with healthcare professionals on updates to care plans and address identified member needs
• Collect, record, and preserve member information and care management activities to ensure regulatory compliance
• Conduct in-person visits to evaluate member needs and work with service providers or resources
• Provide education regarding procedures, healthcare provider instructions, care options, referrals, and healthcare benefits
• Execute other assigned responsibilities while adhering to all policies and standards
• Bachelor's degree with 2–4 years of relevant experience
• Alternatively, equivalent experience gained through the attainment of suitable knowledge, responsibilities, scope, and skills commensurate with this position
• If possessing clinical licensure, must be a graduate of an Accredited School of Nursing
• Current state clinical license is preferred
• Outstanding communication and customer service capabilities
• Experience collaborating with multidisciplinary teams
• Strong problem-solving skills
• Proficient in computer applications
• Effective organizational and time-management abilities
• Capacity to operate independently as a self-starter
• Must reside in Montgomery County, Ohio, or in Butler, Warren, Greene, Clinton, Clark, Miami, or Darke County
• Availability to work Monday through Friday, from 8:00 AM to 5:00 PM
