Care Navigator

This is a fully remote position, open to applicants in California.

📋 Description

• Create, evaluate, and oversee care management activities tailored to the needs of members

• Develop or assist in the creation of individualized care plans or service plans

• Inform members and their families/caregivers about services, benefit options, care alternatives, referrals, and healthcare benefits

• Assess member needs, identify barriers to care, evaluate available resources, and recommend/facilitate plans to achieve optimal outcomes

• Identify necessary providers, specialists, and community resources for care

• Offer psychosocial and resource support related to employment, education, housing, food, participant direction, independent living, justice, and foster care

• Facilitate coordination between members/families/caregivers and care provider teams to ensure timely access to care and services

• Track progress toward care plan goals and monitor any changes in member condition

• Work collaboratively with healthcare providers to update care plans and refer members for further care management evaluation when appropriate

• Gather, document, and maintain member information and care management activities to ensure regulatory compliance

• Conduct on-site visits when necessary

• Execute other responsibilities assigned by the people leader while adhering to policies and standards

⛳️ Requirements

• Bachelor’s degree

• 2–4 years of relevant experience

• If holding clinical licensure, graduation from an Accredited School of Nursing is required

• Equivalent experience gained through applicable knowledge, responsibilities, scope, and skills reflective of this position's level

• Availability to work during Pacific Standard Time (PST) hours

• Must adhere to all policies and standards
