This is a fully remote position, open to applicants in United States.
β’ Develop workflows and processes specifically for veteran care coordination.
β’ Establish guidelines to manage complex cases and escalations effectively.
β’ Determine suitable roles for non-clinical care coordinators, nurses, and social workers.
β’ Identify gaps in programs and explore opportunities for development.
β’ Act as a subject matter expert in VA healthcare, benefits, and services tailored for Veterans.
β’ Assist Care Coordinators in navigating VA, community, and Veteran-specific resources.
β’ Educate Veterans on benefits and connect them with the appropriate resources.
β’ Offer guidance on intricate Veteran cases.
β’ Aid in identifying suitable referrals, resources, and escalation pathways.
β’ Provide direct support to Veterans when necessary.
β’ Collaborate with the Director of Training and Development and the Director of Care Coordination to create training focused on Veterans.
β’ Instruct Care Coordinators on VA healthcare, Veteran resources, benefits navigation, and common challenges.
β’ Serve as a continuous social work/VA subject matter expert for the Care Coordinator team.
β’ Report directly to the Director of Care Coordination.
β’ Operate without supervisory responsibilities.
β’ A minimum of 5 years of experience as a clinical social worker, care coordinator, care navigator, or in a related field.
β’ Proven experience in progressive program management.
β’ At least 1 year of experience in a remote working environment.
β’ Experience working directly with Veterans and/or within the VA system.
β’ Knowledge of VA healthcare, Veteran benefits/resources, behavioral health, and social determinants of health (SDOH).
β’ Experience with medically and/or behaviorally complex populations.
β’ A dedicated home office for remote work is required.
β’ Strong interpersonal, verbal, and written communication skills, along with collaborative and problem-solving abilities.
β’ Experience communicating with members via phone and text messaging.
β’ Proficient in documenting case notes in a care management or electronic health record platform.
β’ Familiarity with motivational interviewing techniques.
β’ Experience with Apple computers is preferred.
β’ Bilingual fluency in Spanish is preferred.
β’ Opportunity to work remotely from a dedicated home office.
