This is a fully remote position, open to applicants in North Carolina.
• Deliver Customized Care Management to individuals on the Registry of Unmet Needs (RUN)
• Engage with members to evaluate their needs
• Coordinate the physical health, behavioral health, and related resource needs of members
• Review applications to determine potential eligibility for Innovations Waiver services
• Inform individuals and families about the Registry of Unmet Needs and the available resources for services, supplies, and equipment
• Assist members in acquiring necessary clinical information
• Support members in initiating relevant Medicaid or disability determination processes
• Travel within the designated catchment area as needed
• Must fulfill Qualified Professional (QP) status
• Fully licensed by the NC governing board overseeing Human Services professions; OR fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four years of full-time experience with the MH/DD/SA population; OR master's degree in a human service field along with one year of full-time MH/DD/SA experience; OR bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; OR bachelor's degree in another field and four years of full-time MH/DD/SA experience
• Alternatives for substance abuse professionals must include the specified supervised experience in alcoholism and drug abuse counseling
• Must possess a valid driver's license
• Residency must be within Trillium's Mid State Region: Anson, Guilford, Montgomery, Randolph, or Richmond counties
• Must be able to travel within the catchment area as needed
• All candidates are required to pass a drug test as a condition of employment
• Resume must detail employer names, dates of service, average weekly hours worked, key job duties, education background, and licensure/certification information if applicable
