This is a fully remote position, open to applicants in United States.
• Act as a telephonic Registered Nurse and Care Manager for CenterWell Home Health.
• Ensure seamless care for patients transitioning into and out of services.
• Provide insights on post-acute care levels and the services offered by CenterWell, including home health, hospice, and palliative care.
• Communicate effectively with the CenterWell Home Health clinical team to facilitate patient follow-ups.
• Support discharged patients and those requiring post-acute services.
• Guide patients through healthcare options, post-acute care services, Medicare coverage, billing inquiries, and healthcare resources.
• Offer patients information on service-site options, specialty services, directions, and related educational materials.
• Address customer issues, complaints, and assist with location searches.
• Assess clinical levels of care using discussions, medical records, and clinical data.
• Align patients with suitable service locations or alternative services.
• Advocate throughout the referral process to enhance referral-to-admission conversion rates.
• Monitor referral and admission outcomes.
• Contribute to departmental performance objectives and patient satisfaction targets.
• Assist with admissions as a patient ambassador.
• Educate about Home Health, Hospice, and Palliative Care Services.
• Aid in clinical eligibility assessments for alternative services.
• Complete required training and adhere to company policies and procedures.
• Engage in special projects and carry out other assigned duties.
• Associate's Degree is mandatory.
• Bachelor of Science in Nursing (BSN) is preferred.
• A compact state RN license is required; additional state licenses may be necessary depending on business requirements.
• A minimum of 3 years of post-acute nursing experience is essential.
• Experience in home health or hospice is preferred.
• Familiarity with home health, hospice, and palliative care services is important.
• Capability to learn and master client locations and services.
• Strong analytical and problem-solving skills.
• Exceptional verbal and interpersonal communication abilities.
• Empathetic phone communication skills.
• Minimum internet speed of 25 Mbps for download and 10 Mbps for upload is required.
• A dedicated workspace free from interruptions to safeguard member PHI/HIPAA information.
• Ability to work a designated 20-hour rotating schedule and holiday shifts.
• Availability for weekday evening and weekend daytime/evening training sessions.
