Care Manager, Telephonic Nurse

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Evaluate and assess the needs and requirements of members in a telephonic setting.

• Direct members and their families toward suitable care and wellbeing resources.

• Address members' physical, environmental, and psycho-social health concerns utilizing diverse strategies and techniques.

• Recognize and eliminate obstacles to effective care delivery.

• Continuously oversee patient care by conducting assessments, analyzing data, engaging in conversations with members, and active care planning.

• Navigate various systems while employing critical thinking skills.

• Conduct health assessments, develop care plans, and provide chronic-condition education via phone.

• Detect deficiencies in members' healthcare plans.

• Manage an assigned population of members.

• Make independent decisions regarding work methodologies and suitable actions while adhering to established guidelines and procedures.

⛳️ Requirements

• A bachelor's degree.

• At least 3 years of clinical acute care experience.

• Licensed Registered Nurse (RN) in Illinois without any disciplinary actions.

• In-depth knowledge of Microsoft Office applications, including Word, Excel, and Outlook.

• A strong desire to contribute to an organization dedicated to enhancing consumer experiences.

• Experience in case management, discharge planning, and patient education for adult acute care.

• Background in managed care.

• Certified Case Manager (CCM) designation.

• Bilingual in English and Spanish.

• Internet service with a minimum download speed of 25 Mbps and an upload speed of 10 Mbps.
