Care Manager, Telephonic Nurse

This is a fully remote position, open to applicants in Arizona, +4 more states.

📋 Description

• Provide patient-centered care over the phone in a structured call-center setting throughout your shift.

• Perform detailed telephonic assessments and evaluations of members.

• Offer education on medications, including administration methods, side-effect management, and support for adherence.

• Create and execute personalized care plans designed to meet the clinical needs of members.

• Track patient progress through continuous assessments and follow-up communications.

• Work in collaboration with established protocols and utilize advanced telecommunication technologies.

• Assist in the Patient Management Program for individuals with specialized and complex health conditions.

• Complete scheduled inbound and outbound tasks as required.

• Available for assigned weekday shifts, late-night Fridays, Saturdays, and holiday rotations.

• Deliver patient support and education for CenterWell Specialty Pharmacy.

⛳️ Requirements

• A Bachelor of Science in Nursing (BSN) is required and must be completed.

• 3–5 years of clinical acute care experience as a Registered Nurse (RN).

• Must hold a valid Registered Nurse (RN) license in the relevant state with no disciplinary actions.

• Residency in OH, KY, FL, AZ, or TX is required.

• Proficient knowledge of Microsoft Office applications, including Word, Excel, and Outlook.

• Experience in managed care is preferred.

• A strong desire to contribute to an organization that is dedicated to continuously enhancing consumer experiences.

• Must demonstrate schedule adherence.

• Preferred: experience in care management.

• Preferred: experience in patient education for adult care.

• Preferred: familiarity with auto dialer systems.

• Preferred: bilingual in English and Spanish.

• A minimum home internet speed of 25 Mbps for downloads and 10 Mbps for uploads is required.

• A dedicated workspace free from interruptions to safeguard member PHI/HIPAA information is essential.

• Overtime may be necessary as required.

• Participation in holiday and Saturday rotations is mandatory.
