Care Manager, Suicide Prevention Program

This is a fully remote position, open to applicants in Nevada.

📋 Description

• Provide virtual assistance to individuals and families dealing with suicidal thoughts and attempts.

• Support outreach, engagement, and ongoing assistance for a group of members.

• Conduct regular check-ins and offer personalized care.

• Develop and coordinate community resources that address medical, behavioral, violence prevention, and social service requirements.

• Evaluate risk and carry out crisis interventions.

• Ensure safety through customized safety plans.

• Collaborate with members to evaluate care options and formulate and update service plans.

• Utilize benefits and community resources effectively.

• Inform members about mental and physical health, abuse prevention, medication, and available resources.

• Act as an advocate between members, families, and providers.

• Assist in scheduling appointments and following up on them.

• Maintain precise records and document case notes in a professional and timely manner.

⛳️ Requirements

• Bachelor’s degree in psychology, social work, or another relevant behavioral health field from an accredited institution.

• Crisis intervention training or a similar certification is mandatory.

• Minimum of 2 years of experience in a related position.

• Preferably have experience with at-risk adolescents and young adults.

• Knowledge of local resources, social determinants of health, mental health, substance abuse disorders, interpersonal violence, and social issues.

• Experience in behavioral health or crisis intervention is essential.

• Strong comprehension of mental health, substance use, and social service resources.

• A compassionate and detail-oriented approach is necessary.
