Care Manager, RN – OB

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, evaluate, and implement intricate care management initiatives for members primarily with physical health needs.

• Assess member requirements, obstacles to accessing care, and social determinants affecting health.

• Create continuous care plans and work in partnership with providers, specialists, and community resources.

• Identify issues and barriers to care, providing appropriate care management interventions.

• Coordinate effectively between members, families/caregivers, and care provider teams.

• Monitor member conditions, changes in health status, and progress towards care plan objectives; adjust plans as necessary.

• Offer resource assistance for employment, housing, participant direction, independent living, justice, and foster care services.

• Facilitate care management to ensure timely access to essential services and care.

• Conduct outreach through telephonic, digital, home visits, and other locations.

• Gather, document, and preserve member information and care management activities to ensure regulatory compliance.

• Educate members and their families/caregivers about disease processes, care gaps, provider instructions, care alternatives, referrals, and healthcare benefits.

• Provide feedback on leadership regarding opportunities to enhance care quality and cost efficiency.

• Adhere to all policies and standards while performing additional assigned responsibilities.

⛳️ Requirements

• Degree from an accredited nursing school or a Bachelor’s degree in Nursing.

• 2–4 years of relevant experience.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.

• Capability to work remotely from any location within the state of Missouri.

• Availability to work Monday–Friday, 8am–5pm.
