Care Manager, RN – OB

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, evaluate, and implement comprehensive care management initiatives for members primarily facing physical challenges.

• Assess member requirements, obstacles to access, and social factors affecting health.

• Create continuous care strategies in partnership with healthcare providers, specialists, and community organizations.

• Identify obstacles to care and deliver suitable care management solutions.

• Coordinate between members, families/caregivers, and care provider teams.

• Track member progress and status in relation to care plan objectives; adjust plans when necessary.

• Offer resource assistance for employment, housing, participant direction, independent living, justice, and foster care services.

• Ensure timely access to essential care and services.

• Conduct outreach through telephonic, digital, home visits, and/or other locations.

• Maintain documentation of member details and care management actions for compliance with regulations.

• Inform members and families/caregivers about disease processes, care gaps, provider recommendations, care alternatives, referrals, and healthcare benefits.

• Provide leadership insights on areas for enhancing care quality and cost-efficiency.

• Adhere to all policies and standards while performing additional assigned tasks.

⛳️ Requirements

• A degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience.

• RN (Registered Nurse) state licensure and/or compact state licensure is mandatory.

• Availability to work Monday–Friday, from 8 am to 5 pm.

• Must be a resident of Missouri.
