Care Manager, RN

This is a fully remote position, open to applicants in United States.

📋 Description

• Design, evaluate, and implement intricate care management activities primarily for members with physical health needs.

• Perform outreach to members and oversee case management enrollment.

• Continuously manage members and engage in clinical rounds.

• Assess member needs, identify barriers to care, and consider social determinants of health.

• Create care plans and collaborate with healthcare providers, specialists, and community resources.

• Recognize care barriers and deliver suitable care management interventions.

• Coordinate person-centered care among members, families, caregivers, and healthcare teams.

• Monitor and follow up on member status, changes in condition, and progress toward care plan objectives.

• Offer resource support for employment, housing, self-direction, independent living, justice, and foster care services.

• Facilitate access to necessary care and services.

• Conduct outreach via telephone, digital platforms, home visits, and/or other locations.

• Gather, document, and maintain member information and care management activities to ensure regulatory compliance.

• Provide education to members and families/caregivers regarding disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.

• Offer leadership insights on opportunities to enhance care quality and cost-effectiveness.

• Adhere to all policies and standards while performing additional assigned duties.

⛳️ Requirements

• Degree from an accredited nursing school or a Bachelor’s degree in Nursing.

• 2–4 years of relevant experience.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.

• Experience in disease management.

• Strong clinical judgment skills.

• Proficiency in Microsoft applications.

• Experience in physical health case management.

• Hospice or home health experience is preferred.

• Maternity experience is advantageous.
