Care Manager, RN

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Design, implement, and oversee intricate care management activities for members primarily with physical health requirements.

• Develop tailored care plans while educating members and their families.

• Assess member needs, obstacles to care, and social determinants affecting health.

• Create ongoing care/service plans and collaborate with healthcare providers, specialists, and community resources.

• Recognize issues and obstacles to care and implement suitable interventions.

• Coordinate care among members, their caregivers, and provider teams.

• Monitor member conditions, changes in status, and progress towards care-plan objectives; adjust plans as necessary.

• Offer resource support for employment, housing, participant direction, independent living, justice, and foster care services.

• Facilitate care management and ensure timely access to essential care and services.

• Conduct outreach through telephonic, digital, home, and/or various site interactions.

• Collect, document, and maintain member information and care management activities to ensure regulatory compliance.

• Educate members and caregivers about disease processes, care gaps, provider directives, care options, referrals, and healthcare benefits.

• Provide leadership insights on opportunities to enhance care quality and cost-effectiveness.

• Execute other assigned tasks and adhere to all policies and standards.

⛳️ Requirements

• Candidates must be residents of South Carolina.

• An active RN license is required.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.

• A degree from an Accredited School of Nursing or a Bachelor's degree in Nursing is essential.

• 2–4 years of relevant experience is required.

• Proficiency in computer skills is necessary.

• Outstanding communication and customer service skills are essential.

• Strong assessment and critical-thinking abilities are required.

• Experience in care management is necessary.

• Effective time-management skills are a must.

• Keen attention to detail is important.

• Availability from Monday to Friday, 8:00 AM to 5:00 PM is required.
