This is a fully remote position, open to applicants in New Jersey.
• Design, evaluate, and oversee intricate care management activities for members with predominantly physical requirements.
• Develop tailored care plans and provide education to members and their families.
• Assess member needs, identify access obstacles, and consider social determinants of health.
• Create continuous care/service strategies and collaborate with providers, specialists, and community resources.
• Recognize care obstacles and implement care management interventions.
• Coordinate person-centered care among members, caregivers, and provider teams.
• Monitor and follow up on member status, changes in condition, and progress of care plans.
• Offer local resource support for services related to employment, housing, participant direction, independent living, justice, and foster care.
• Ensure prompt access to necessary care and services.
• Conduct outreach via telephone, digital platforms, home visits, and other locations.
• Gather, document, and manage member information and care management activities to comply with regulatory standards.
• Inform members and caregivers about disease processes, care gaps, provider instructions, care alternatives, referrals, and healthcare benefits.
• Provide leadership insights on opportunities to enhance care quality and cost efficiency.
• Carry out additional assigned tasks and adhere to policies and standards.
• Degree from a recognized nursing school or a Bachelor’s degree in Nursing.
• 2–4 years of relevant experience.
• Current New Jersey Registered Nurse (RN) license is mandatory.
• State Licensure and/or Compact State Licensure is required.
• Capable of conducting up to 50% local travel for in-home member visits within the assigned area.
• Availability from Monday to Friday, 8:00 a.m. to 5:00 p.m. ET, with flexibility according to member and business requirements.
