Care Manager, RN

This is a fully remote position, open to applicants in Mississippi.

📋 Description

• Design, evaluate, and implement intricate care management activities for members with predominantly physical health needs.

• Assess member requirements, obstacles to access, and social determinants impacting health.

• Create continuous care/service plans and work in partnership with providers, specialists, and community organizations.

• Recognize care challenges and barriers, offering appropriate care management interventions.

• Facilitate communication among members, families/caregivers, and care provider teams.

• Monitor members' conditions, changes in status, and progress toward care plan objectives; adjust plans as necessary.

• Offer support resources for employment, housing, participant direction, independent living, justice, and foster care services.

• Ensure prompt access to required care and services.

• Conduct outreach via telephonic, digital, home, and other locations.

• Collect, document, and preserve member data and care management activities to comply with regulatory standards.

• Educate members and their families/caregivers about disease processes, care gaps, provider instructions, care alternatives, referrals, and healthcare benefits.

• Provide feedback on leadership opportunities to enhance care quality and cost-effectiveness.

• Carry out other assigned responsibilities and adhere to established policies and standards.

⛳️ Requirements

• Applicants must be residents of Mississippi.

• A current, unrestricted Registered Nurse (RN) license in Mississippi is mandatory.

• Degree from an Accredited School of Nursing or a Bachelor’s degree in Nursing is required.

• 2 to 4 years of relevant experience is necessary.

• Prior experience in case management and discharge planning is highly preferred.

• RN state licensure and/or compact state licensure is required.

• Work schedule is Monday through Friday, from 8:00 AM to 5:00 PM CST.
