Care Manager Registered Nurse – Field

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Collaborate and contribute to the case management process by utilizing critical thinking and sound judgment.

• Facilitate optimal healthcare outcomes for members.

• Assist in scheduling appointments.

• Identify and support members in accessing their benefits and educational resources.

• Utilize care management tools and resources effectively.

• Serve members who are dual-eligible for Medicare and Medicaid, facing complex health and social challenges.

• Work in partnership with members, healthcare providers, and community organizations.

• Address the healthcare and social determinant needs of members.

• Travel across East Central Illinois, including the counties of Champaign, Vermilion, Ford, Piatt, Douglas, Coles, and Edgar.

• Operate from a designated home virtual workspace while collaborating with teams remotely.

⛳️ Requirements

• Candidates must reside in the state of Illinois.

• Reliable transportation is required, with a willingness and ability to travel 50-75% of the time from the home location within East Central Illinois.

• A minimum of 3-5 years of practical clinical experience is necessary.

• Must be confident in working from home as an independent thinker, utilizing tools for virtual collaboration and connection with teams.

• Excellent analytical and problem-solving abilities are essential.

• Strong communication, organizational, and interpersonal skills are required.

• Capability to work independently is a must.

• Proficient computer skills, including the ability to navigate multiple systems and keyboarding.

• Familiarity with standard corporate software applications such as MS Word, Excel, Outlook, and PowerPoint, along with proprietary applications.

• An Associate's degree is required.

• An active and unencumbered Registered Nurse license in Illinois is necessary.

• A direct/hardwired internet connection with a minimum of 25 Mbps download and 3 Mbps upload is required; WiFi and satellite internet are not acceptable.

• A quiet, secure, and private designated home virtual work location that complies with CVS Health and HIPAA guidelines is required.

• Must work within the state and city of current residence.

• Candidates must provide their own workspace furniture.

• Ability to use multiple monitors and navigate through various applications is necessary.

• Must be able to communicate via email, calendar invites, Teams messaging, and virtual meetings.

• Experience with Microsoft Office 365 applications or similar Google Workspace is required.

• Ability to log in to secure systems such as VPN and EHR portals, maintain security practices, and recognize suspicious emails or links is essential.

• Capability to resolve common technical issues independently is necessary.

• Openness to learning new skills is required.
