Care Manager, Registered Nurse - Field

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Employ critical thinking and sound judgment to support and enhance the case management process.

• Promote favorable healthcare outcomes for members.

• Assist with scheduling appointments.

• Identify and aid members in accessing their benefits and educational resources.

• Utilize care management tools and resources effectively.

• Collaborate with members, healthcare providers, and community organizations.

• Address both healthcare and social determinant needs of members.

• Travel up to 75% across Metro East/Southern Illinois and adjacent counties.

• Operate independently from home while maintaining virtual connections with teams.

⛳️ Requirements

• Must be a resident of Illinois.

• Must have reliable transportation and be prepared to travel up to 75% of the time from the home location in and around Metro East/Southern Illinois.

• Minimum of 3-5 years of clinical practical experience is required.

• Must be confident working from home as an independent thinker, utilizing tools to collaborate and connect with teams virtually.

• Strong analytical and problem-solving abilities are essential.

• Excellent communication, organizational, and interpersonal skills are necessary.

• Ability to work autonomously.

• Proficient computer skills are required, including navigating multiple systems and keyboarding.

• Proficiency in MS Word, Excel, Outlook, PowerPoint, and proprietary applications is mandatory.

• An Associate's degree is required.

• Must hold an active and unencumbered Registered Nurse license in Illinois.

• A direct/hardwired internet connection with a minimum of 25 Mbps download and 3 Mbps upload is required; WiFi and satellite internet are not acceptable.

• A quiet, secure, and private designated home workspace must comply with CVS Health and HIPAA guidelines.

• Must operate within the state and city of current residence.

• Must supply personal workspace furniture.

• Capability to use multiple monitors and applications simultaneously is essential.

• Proficiency in digital communication tools and virtual meeting platforms is necessary.

• Experience with Microsoft Office 365 or similar Google Workspace is preferred.

• Ability to access secure systems such as VPN and EHR portals while adhering to security protocols is required.

• Must be able to resolve common technical issues independently.

• Preferred: 2-3 years experience in Care Management, discharge planning, and/or home health care coordination.

• Preferred: Certified Case Manager certification.

• Bilingual candidates are preferred.
