Care Manager, Registered Nurse - Field

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Collaborate and contribute to the case management process utilizing critical thinking and judgment.

• Facilitate optimal healthcare outcomes for members.

• Assist with scheduling appointments.

• Identify and support members in accessing benefits and educational resources.

• Utilize care management tools and resources effectively.

• Work collaboratively with members, healthcare providers, and community organizations.

• Address the healthcare and social determinant needs of members across the entire continuum of care.

• Travel within the North/Northwest Suburbs of Chicago for field-based work, while working from home when not traveling.

⛳️ Requirements

• Must reside in the state of Illinois.

• Must have reliable transportation and be willing to travel up to 50-75% of the time from the home location in and around the North/Northwest Suburbs of Chicago.

• Minimum of 3-5 years of clinical practical experience required.

• Confidence in working from home and being an independent thinker, utilizing tools for virtual collaboration and connection with teams.

• Excellent analytical and problem-solving capabilities.

• Strong communication, organizational, and interpersonal skills.

• Ability to work independently.

• Proficient computer skills, including the ability to navigate multiple systems and proficient keyboarding.

• Competence in standard corporate software applications, including MS Word, Excel, Outlook, PowerPoint, and proprietary applications.

• Associate's degree is required; Bachelor's degree is preferred.

• Must hold an active and unencumbered Registered Nurse license in the state of Illinois.

• Direct/hardwired internet connection to a modem/router within 7 feet of the computer, with a minimum of 25 Mbps download and 3 Mbps upload speeds.

• WiFi and satellite internet are not permitted.

• A quiet, secure, and private designated home virtual work location compliant with CVS Health and HIPAA guidelines is necessary.

• Must work within the state and city of current residence.

• Must provide personal workspace furniture.

• Experience in setting up and using multiple monitors and navigating several applications simultaneously.

• Ability to communicate through email, calendar invites, Teams messaging, and virtual meetings.

• Familiarity with Microsoft Office 365 applications or similar Google Workspace applications is preferred.

• Capability to log in to secure systems such as VPN and EHR portals, manage strong passwords, and identify suspicious emails or links.

• Ability to independently resolve common technical issues.

• Preferred: 2-3 years of experience in Care Management, discharge planning, and/or home health care coordination.

• Preferred: Certified Case Manager designation.

• Bilingual candidates are preferred.
