Care Manager – RCT

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Oversee and/or engage with members participating in Tailored Care Management, providing necessary support.

• Conduct thorough assessments of members' needs and analyze the outcomes.

• Develop and uphold integrated care plans that address physical health, mental well-being, social support, lifestyle choices, community engagement, and risk factors affecting well-being.

• Organize regular in-person and telephonic meetings with members.

• Consult with healthcare providers to collect and disseminate information pertinent to members' care.

• Act as the primary point of contact for members.

• Collaborate with multidisciplinary care teams, including primary care physicians, specialists, care manager extenders, transition staff, and caregivers/legal guardians.

• Work remotely from 8:30 a.m. to 5:00 p.m., travel within the community to meet members, and attend meetings as necessary.

⛳️ Requirements

• Must fulfill Qualified Professional (QP) criteria.

• Fully licensed by the North Carolina governing body overseeing Human Services professions; OR a fully licensed RN permitted to practice in North Carolina by the NC Board of Nursing with four years of full-time experience in the MH/DD/SA population; OR possess a qualifying master's degree in a human service field with one year of full-time relevant experience; OR hold a bachelor's degree in a human service field with two years of full-time relevant experience; OR have a bachelor's degree in a different field with four years of full-time relevant experience.

• Substance abuse professionals must possess the required supervised experience in alcoholism and drug abuse counseling.

• For those serving LTSS members, a minimum of two years' experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management is required.

• Must possess a valid driver's license.

• Must reside within Trillium's Mid State Region: Anson, Guilford, Montgomery, Randolph, or Richmond counties.

• Must be available to travel within the designated area as required.

• All applicants must successfully pass a drug test as a condition of employment.
