This is a fully remote position, open to applicants in North Carolina.
• Act as the main contact for designated members, offering continuous care management and advocacy.
• Perform thorough assessments of member requirements utilizing person-centered planning techniques and organizational resources.
• Create, execute, and oversee personalized care plans in collaboration with members, families, and multidisciplinary support teams.
• Facilitate services among providers, agencies, and community resources to meet physical, behavioral, and social health requirements.
• Ensure that assessments and care plans are updated to reflect shifts in member needs and situations.
• Collaborate with primary care providers, specialists, care manager extenders, transition staff, and caregivers/legal guardians.
• Maintain remote availability from 8:30 a.m. to 5:00 p.m., travel within the community to meet with members, and participate in meetings as necessary.
• Must fulfill Qualified Professional (QP) criteria.
• Fully licensed by the NC governing board overseeing Human Services professions; OR fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four years of full-time experience in the MH/DD/SA population; OR a master's degree in a human service field with one year of full-time experience in MH/DD/SA; OR a bachelor's degree in a human service field with two years of full-time experience in MH/DD/SA; OR a bachelor's degree in another discipline with four years of full-time experience in MH/DD/SA.
• Substance abuse professional alternatives necessitate supervised experience in alcoholism and drug abuse counseling as defined.
• If working with LTSS members, a minimum of two years of previous LTSS and/or HCBS coordination, care delivery monitoring, and care management experience is required.
• Must possess a valid driver's license.
• Must live within 75 miles of Trillium's South Central Region: Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, or Wayne counties.
• Must be capable of traveling within the catchment area as needed.
• All applicants must pass a drug test as a prerequisite for employment.
