This is a fully remote position, open to applicants in North Carolina.
• Act as the main contact for designated members.
• Provide continuous care management and advocacy.
• Perform thorough member-needs evaluations employing person-centered planning techniques and organizational tools.
• Create, execute, and oversee personalized care plans.
• Collaborate with members, their families, and multidisciplinary support teams.
• Coordinate services among various providers, agencies, and community resources.
• Address the physical, behavioral, and social health requirements of members.
• Revise assessments and care plans as members' needs and situations evolve.
• Engage with primary care providers, specialists, care manager extenders, transition staff, and caretakers/legal guardians.
• Maintain remote availability from 8:30 a.m. to 5:00 p.m., travel within the community to meet with members, and attend meetings as necessary.
• Must fulfill Qualified Professional (QP) criteria.
• Fully licensed by the NC governing board that regulates Human Services professions; OR fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with a minimum of four years’ experience in the MH/DD/SA population; OR a master's degree in a human service field along with one year of full-time experience in MH/DD/SA; OR a bachelor's degree in a human service field with two years of full-time experience in MH/DD/SA; OR a bachelor's degree in a different field combined with four years of full-time experience in MH/DD/SA.
• Substance abuse professional alternatives necessitate the specified supervised experience in alcoholism and drug abuse counseling.
• If serving LTSS members, at least two years of previous experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management is required.
• Must possess a valid driver's license.
• Must live within Trillium's Southern Region: Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, or Wayne County, North Carolina.
• Must be able to travel within the designated area as needed.
• Must successfully pass a drug test as a prerequisite for employment.
• Resume must detail employer names, dates of employment, average weekly hours, key responsibilities, educational background, and licensure/certification details.
• Preferred: degree in health, psychology, sociology, social work, nursing, or another relevant human services field.
