This is a fully remote position, open to applicants in North Carolina.
β’ Oversee, engage, support, and conduct outreach for members enrolled in Tailored Care Management.
β’ Conduct thorough assessments to identify members' needs and analyze outcomes.
β’ Develop and uphold integrated care plans that encompass physical health, mental health, social support, lifestyle, community involvement, and well-being risks.
β’ Act as the primary contact for members.
β’ Collaborate with multidisciplinary care teams, including providers, specialists, care manager extenders, transition personnel, and caretakers/legal guardians.
β’ Organize regular in-person and telephonic meetings with members.
β’ Consult with providers to collect and disseminate relevant information regarding members' care.
β’ Work remotely from 8:30 am to 5:00 pm and travel within the community to meet members and participate in meetings as necessary.
β’ Must fulfill Qualified Professional (QP) criteria.
β’ Fully licensed by the North Carolina governing board overseeing Human Services professions, or possess a valid RN license to practice in North Carolina as per the NC Board of Nursing.
β’ RN pathway necessitates four years of full-time experience with the MH/DD/SA population served.
β’ A master's degree in a human service field along with one year of full-time experience in MH/DD/SA, or one year of supervised counseling experience in alcoholism and drug abuse.
β’ A bachelor's degree in a human service field plus two years of full-time MH/DD/SA experience, or two years of supervised counseling experience in alcoholism and drug abuse.
β’ A bachelor's degree in a different field accompanied by four years of full-time MH/DD/SA experience, or four years of supervised counseling experience in alcoholism and drug abuse.
β’ If working with LTSS members, a minimum of two years of previous experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management.
β’ Must possess a valid driver's license.
β’ Residency in Trillium's North Central Region: Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, or Wilson counties is required.
β’ Ability to travel within the designated catchment area as needed.
β’ Successful completion of a drug test is a prerequisite for employment.
