Care Manager – RB-BHT

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver intricate care coordination and monitoring for members with primary intellectual or developmental disabilities, along with potential secondary mental health, substance use, or complex medical needs.

• Facilitate integrated care management through collaboration with multidisciplinary teams and referrals.

• Support the TCM opt-in/opt-out process.

• Conduct care needs screenings, develop 90-day transition plans, CMCAs, and care plans, ensuring updates as necessary.

• Coordinate services as needed and appropriate.

• Act as the main point of contact for assigned members, offering continuous care management and advocacy.

• Perform comprehensive, person-centered assessments of member needs.

• Create, implement, and oversee individualized care plans in collaboration with members, families, and multidisciplinary support teams.

• Coordinate services among providers, agencies, and community resources.

• Revise assessments and care plans as member needs and circumstances evolve.

• Primarily focus on members diagnosed with Autism Spectrum Disorder, especially children aged 18 months to 15 years.

• Work remotely from 8:30 am to 5:00 pm while also traveling within the community to meet with members and attend meetings.

⛳️ Requirements

• Must qualify as a Qualified Professional (QP).

• Fully licensed by the NC governing board that regulates Human Services professions; OR a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four years of full-time experience serving the MH/DD/SA population.

• A Master's degree in a human service field and one year of full-time MH/DD/SA experience, or one year of supervised experience in alcoholism and drug abuse counseling.

• A Bachelor's degree in a human service field and two years of full-time MH/DD/SA experience, or two years of supervised experience in alcoholism and drug abuse counseling.

• A Bachelor's degree in a different field and four years of full-time MH/DD/SA experience, or four years of supervised experience in alcoholism and drug abuse counseling.

• If serving LTSS members, a minimum of two years of prior experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management.

• A valid driver's license is required.

• Must reside within Trillium's South Central or Mid State region: Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne, Anson, Guilford, Montgomery, Randolph, or Richmond counties.

• Must be able to travel within the catchment area as necessary.

• Must pass a drug test as a condition of employment.
