Care Manager – LPN

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee a panel of patients participating in Chronic Care Management (CCM), Principal Care Management (PCM), Transitional Care Management (TCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI), and various other care management programs.

• Perform scheduled telephonic outreach to evaluate patient needs, symptoms, medication adherence, and barriers to accessing care.

• Reinforce personalized care plans and provide education on chronic disease management, medications, preventive measures, and healthy lifestyle choices.

• Identify gaps in care and coordinate follow-up appointments, screenings, laboratory work, and referrals.

• Evaluate social determinants of health and connect patients with relevant community resources.

• Detect changes in patient conditions and escalate concerns to the appropriate provider according to established protocols.

• Review medication lists, identify potential issues, and communicate findings to the care team.

• Assist patients following hospitalizations, emergency department visits, or other significant health events.

• Accurately document patient interactions, interventions, and care management activities within the electronic health record.

• Collaborate with providers, pharmacists, behavioral health professionals, specialists, and care management teams.

• Uphold productivity, quality, documentation, and compliance standards.

• Participate in virtual huddles, case reviews, training sessions, and quality improvement initiatives.

⛳️ Requirements

• Graduate from an accredited Practical Nursing program.

• Hold an active, unrestricted Florida LPN license with Nurse Licensure Compact (NLC) multistate privileges.

• Possess a current Compact (multistate) nursing license.

• Have a minimum of 1 year of clinical nursing experience.

• Preferred experience in primary care, chronic disease management, home health, population health, or related clinical settings.

• Strong skills in assessment, communication, organization, and documentation.

• Comfortable working independently in a remote setting while collaborating with a multidisciplinary team.

• Previous experience in remote or telephonic nursing is preferred.

• Preferred experience with CCM, RPM, Medicare, Medicaid, or value-based care.

• Familiarity with Athenahealth, ChronicCareIQ, 3CX, or similar healthcare platforms is preferred.

• Basic Life Support (BLS) certification is preferred.

• Bilingual (Spanish) is a plus.

• Must reside in Florida.

• Have a private, secure home office with reliable high-speed internet.

• Ability to maintain HIPAA compliance while working remotely.

• Availability and responsiveness during scheduled work hours.

• Background checks are mandatory for this position.
