This is a fully remote position, open to applicants in United States.
• Deliver comprehensive care management services for patients participating in APCM, CCM, and other value-based care initiatives.
• Analyze electronic health records to pinpoint gaps in care for patients living in long-term care nursing facilities.
• Review and authorize initial and ongoing health questionnaires completed by care management team members.
• Conduct regular monthly telephonic outreach to patients and/or their caregivers.
• Create patient-centered care plans.
• Inform patients and durable medical power of attorney representatives about the benefits of APCM or CCM.
• Aid in closing quality gaps through clinical discovery.
• Coordinate orders, referrals, and prior authorizations with the virtual care support team.
• Escalate abnormal diagnostic test results to Curana providers.
• Work collaboratively with providers to ensure coordinated, patient-centered care.
• Relay patient health updates to Curana providers.
• Communicate treatment plans and health updates to caregivers.
• Act as the primary liaison between providers and the administrative support team.
• Perform additional duties as assigned.
• Must possess an active, unrestricted Compact LPN license.
• Capability to acquire additional state licenses as necessary.
• Minimum of 2 years of nursing experience required; care settings may encompass inpatient, outpatient, or skilled nursing facilities.
• Understanding of pathophysiology and standard treatment protocols for prevalent health diagnoses, including diabetes, chronic heart failure, and chronic obstructive pulmonary disease.
• Ability to evaluate patient records to uncover gaps in care and report findings to the provider.
• Capacity to work in a remote environment that is devoid of distractions.
• Strong computer skills and adaptability to various technology platforms.
• Exceptional written communication skills.
• Proven experience utilizing clinical data to inform decision-making.
• Ability to work independently as well as part of the interdisciplinary care team.
• Prior experience in Chronic Care Management (CCM) and/or care management is preferred.
• CCM certification is highly preferred.
• Experience with Electronic Health Records is preferred.
• Reliable high-speed internet connection is necessary.
