This is a fully remote position, open to applicants in North Carolina.
• Deliver comprehensive care coordination and monitoring for individuals with primary intellectual or developmental disabilities, along with potential secondary behavioral health or complex medical/physical health needs.
• Employ person-centered planning techniques to collect information, comprehend members' requirements, and address specific needs.
• Educate and support members regarding their rights and responsibilities.
• Conduct discovery activities, gather information, and perform assessments as a part of the planning process.
• Ensure that assessments and plans are revised as necessary and when members' situations evolve.
• Maintain high-quality support and comply with waiver requirements.
• Facilitate integrated care management through collaboration with multidisciplinary teams.
• Travel within Trillium's designated catchment area as needed.
• Must possess QP status.
• Fully licensed by the NC governing board overseeing Human Services professions; OR a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four years of full-time experience in the MH/DD/SA population; OR a master's degree in a human service field with one year of full-time MH/DD/SA experience; OR a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; OR a bachelor's degree in another discipline with four years of full-time MH/DD/SA experience.
• Alternatives for substance abuse professionals require the specified supervised experience in alcoholism and drug abuse counseling.
• For members requiring LTSS, a minimum of two years of previous LTSS and/or HCBS coordination, care delivery monitoring, and care management experience is essential; this may be concurrent with QP experience.
• Licensure and/or certification may be acknowledged as specified above.
• A valid driver's license is required.
• Candidates must reside within Trillium's North Carolina catchment, which includes eligible counties such as Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, and Wilson.
• Must be capable of traveling within the catchment as required.
