Care Manager, FT, 11p-7am

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Support residents in all facets of daily living as specified in their care plan, which includes personal care, food service, housekeeping, laundry, behavior management, social interaction, and activities.

• Offer assistance with bathing, dressing, toileting, and feeding.

• Uphold resident safety, comfort, dignity, and confidentiality at all times.

• Accompany residents to meals and various activities.

• Ensure that assigned residents are clean, well-dressed, groomed, safe, and fed throughout the assigned shifts.

• Record all services rendered during the shift accurately.

• Promptly notify the supervisor of any changes in a resident’s condition.

• Report directly to the Clinical Services Director.

⛳️ Requirements

• Valid Certified Nursing Assistant and/or DHS certification along with a High School Diploma.

• Knowledge and experience in caring for the elderly population.

• Experience with dementia care is preferred.

• Capability to stand, walk, sit, utilize fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.

• Ability to lift or carry items weighing up to 50 lbs.

• Capacity to lift and support residents intermittently as needed.

• Must adhere to proper body mechanics.
