This is a fully remote position, open to applicants in United States.
• Oversee and assist patients with their healthcare requirements through telephonic outreach.
• Perform and document visits utilizing various technology platforms and electronic health records (EHRs).
• Track adherence to care plans and assess their effectiveness.
• Monitor patient progress promptly and implement changes as necessary.
• Facilitate patient access to suitable medical and specialty providers.
• Educate patients and their family/caregivers about pertinent community resources.
• Coordinate the continuity of patient care with external healthcare organizations and facilities.
• Manage transitions from primary care providers to specialty care providers.
• Support patients in self-managing their diseases and modifying their behaviors.
• Provide health counseling, education, and instruction to patients.
• Utilize computer applications effectively.
• Make and receive phone calls for the majority of the work shift.
• Greet patients upon arrival.
• Review patient medical records thoroughly.
• Code and complete insurance documentation.
• Schedule patient appointments.
• Organize hospital admissions and laboratory services.
• Manage correspondence, billing, and bookkeeping activities.
• Escalate patient concerns or needs to the relevant provider.
• Carry out additional tasks necessary to uphold company values and achieve its mission.
• Certified Medical Assistant (CMA) Certification.
• Maintaining certification is an ongoing requirement for this position.
• A minimum of 1 year of experience in a healthcare environment dealing with patients with complex chronic conditions is preferred.
• Experience in Chronic Care Management is required (CCM, PCM, APCM).
• Proficient in using a personal computer.
• Possess technical and analytical skills.
• Excellent communication, organizational, and interpersonal abilities.
• Familiarity with Microsoft Excel is preferred.
• Capable of working remotely from home in a private HIPAA-compliant setting.
• Must have the capacity to house company equipment necessary for the job.
• Must have broadband Internet access.
• Internet download speed should be at least 24 mbps and upload speed at least 4 mbps.
• Must successfully pass a background check, drug screening, and identity verification process prior to employment.
• Immigration or work visa sponsorship will not be available.
• Ability to hear in the normal range and use a headset/earpiece.
• Good visual acuity to read computer screens, scripts, forms, etc.
• May need to sit for 100% of the time while taking calls.
• Accessing the electronic medical record (EMR) system may require the use of a personal mobile device for authentication.
