Care Manager – Behavioral Health

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Create, evaluate, and implement intricate care management strategies primarily addressing mental and behavioral health issues.

• Deliver high-quality, cost-efficient healthcare results through tailored care plans and education for members and their families.

• Assess member requirements via phone consultations or in-home visits and recommend or facilitate appropriate care/service plans.

• Conduct outreach through telephonic, digital, home, and various site-visit methods.

• Formulate continual care plans for members with significant health challenges.

• Identify providers, specialists, and community resources addressing mental health and substance use disorders.

• Coordinate efforts among members, families/caregivers, community resources, and care provider teams.

• Oversee care plans, member status, treatment side effects, complications, and clinical symptoms.

• Support care coordination and ensure timely access to necessary care and services.

• Gather, document, and maintain member information and care management activities to ensure regulatory compliance.

• Inform members and families about procedures, treatment options, referrals, provider instructions, and healthcare benefits.

• Provide constructive feedback on opportunities to enhance care quality and cost efficiency.

• Carry out other assigned duties as required.

⛳️ Requirements

• Master's degree in Behavioral Health or Social Work, or a degree from an Accredited School of Nursing.

• 2–4 years of relevant experience.

• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN in accordance with state contract requirements.

• Proven experience in behavioral health is essential.

• Capability to work remotely from anywhere within the state of Missouri.

• Availability from Monday to Friday, 8 am to 5 pm.
