Care Manager

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver proactive intervention and coordination of care for eligible Vaya Health members and recipients.

• Perform thorough Health Risk Assessments that encompass behavioral, developmental, medical, social, and resource needs.

• Administer screenings such as PHQ-9, GAD, CRAFT, ACES, LOCUS/CALOCUS, and others within the defined scope.

• Create, update, and oversee person-centered care plans and crisis plans.

• Connect members with both formal and informal services and supports across various health domains.

• Organize interdisciplinary care team meetings with members, caregivers, providers, RNs, pharmacists, and other stakeholders.

• Assist with transitional care management and diversion from institutional placements.

• Provide crisis intervention, education, self-management strategies, referrals, and follow-up care.

• Evaluate provider assessments and consult with clinical staff as necessary.

• Oversee service provision, member health and safety, critical incidents, eligibility, and care gaps.

• Lead and facilitate high-risk team meetings involving DSS, DJJ, CCNC, schools, and community stakeholders.

• Accurately document care management activities while maintaining AHR compliance.

• Collaborate with Vaya departments and community partners to pinpoint and resolve system barriers.

• Engage in multidisciplinary huddles, complex case staffing, training sessions, projects, and other assigned responsibilities.

⛳️ Requirements

• A Bachelor’s degree is required, ideally in health, psychology, sociology, social work, nursing, or another relevant area of human services.

• A minimum of two years of experience directly working with individuals who have behavioral health conditions.

• At least two years of previous experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management.

• North Carolina Qualified Professional status in accordance with the population served under 10A NCAC 27G .0104.

• If holding a Bachelor’s degree in nursing and RN, must possess a license from the North Carolina Board of Nursing to practice in North Carolina.

• Residency in North Carolina or within 40 miles of the North Carolina border is required.

• Capability to drive and remain seated for extended periods, including in rural areas.

• Intermediate to advanced proficiency in Microsoft Office products and Vaya systems.

• Familiarity with the DSM and the MH/SU/IDD/TBI service array.

• Ability to complete and uphold all required Vaya trainings and proficiencies.

• Strong communication skills, both clear and effective.

• Proficient interpersonal, organizational, problem-solving, negotiation, arbitration, and conflict-resolution abilities.

• Capacity to make quick independent decisions and adapt to shifting priorities.

• Preferred experience in working directly with individuals with I/DD or TBI.

• Experience and understanding of NC Medicaid programs, Medicaid Transformation, Tailored Plans, state-funded services, and accreditation requirements are preferred.
