This is a fully remote position, open to applicants in North Carolina.
• Deliver comprehensive, whole-person care management to members within a managed care setting.
• Act as the primary contact for assigned members, offering continuous care management and advocacy.
• Perform thorough assessments of member needs utilizing person-centered planning techniques and organizational tools.
• Create, execute, and monitor tailored care plans in collaboration with members, their families, and multidisciplinary support teams.
• Coordinate services among providers, agencies, and community resources to meet physical, behavioral, and social health requirements.
• Ensure that assessments and care plans are revised to reflect updates in member needs and situations.
• Maintain compliance with regulatory and organizational standards.
• Travel within the designated catchment area as necessary.
• Must fulfill QP status.
• Fully licensed by the NC governing board overseeing Human Services professions; OR a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four years of full-time experience with the MH/DD/SA population served; OR a master's degree in a human service field with one year of full-time MH/DD/SA experience or qualifying supervised substance-abuse counseling experience; OR a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience or qualifying supervised substance-abuse counseling experience; OR a bachelor's degree in another field with four years of full-time MH/DD/SA experience or qualifying supervised substance-abuse counseling experience.
• If serving members with LTSS needs, must also have a minimum of two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience; this may be concurrent with QP experience.
• Licensure and/or certification may be accepted as outlined above.
• Must possess a valid driver's license.
• Must reside in North Carolina and within Trillium's South Central Region or within 75 miles, including Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, and Wayne counties.
• Must be able to travel within the catchment area as necessary.
• All candidates must successfully pass a drug test as a condition of employment.
