This is a fully remote position, open to applicants in North Carolina.
• Deliver comprehensive care coordination and oversight for members with primary Intellectual or Developmental Disabilities, as well as potential secondary mental health, substance use, or intricate medical/physical health requirements.
• Perform assessments, monitoring, and intricate care coordination to guarantee quality support and compliance with waiver stipulations.
• Facilitate integrated care management through collaboration with multidisciplinary teams and appropriate referrals.
• Employ person-centered planning techniques and strategies to collect information and address individual needs effectively.
• Provide education and support to members regarding their rights and responsibilities, encouraging them to exercise these rights.
• Conduct Discovery activities, including information collection and assessment, as a key part of the planning process.
• Inform members and their legally responsible parties about Medicaid, state-funded services, service choices, requirements, and available natural and community resources.
• Assist members and their legally responsible parties in selecting service providers through an objective lens.
• Serve children, adolescents, and adults who have Intellectual/Developmental Disabilities.
• Must fulfill QP status.
• Fully licensed by the NC governing board overseeing Human Services professions; OR possess a fully licensed RN credential to practice in North Carolina issued by the NC Board of Nursing alongside four years of full-time MH/DD/SA experience; OR hold a master's degree in a human services field with one year of full-time MH/DD/SA experience; OR possess a bachelor's degree in a human services field with two years of full-time MH/DD/SA experience; OR have a bachelor's degree in a different field alongside four years of full-time MH/DD/SA experience.
• If working with members requiring LTSS, at least two years of prior experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management is essential.
• A valid driver's license is required.
• Candidates must reside in North Carolina and be within 75 miles of Trillium's South Central Region.
• Eligible counties in the South Central Region include Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, and Wayne.
• Willingness to serve all counties in the region, including Duplin, Sampson, and Pender counties, is necessary.
• Must be able to travel within the designated catchment area as needed.
• Preferred: Bachelor's degree in human services.
• Preferred: Experience with the Innovation Waiver or IDD population along with skills in writing person-centered plans.
