This is a fully remote position, open to applicants in North Carolina.
• Provide comprehensive care coordination and monitoring for members with primary Intellectual or Developmental Disabilities, along with potential secondary mental health, substance abuse, or complex medical/physical health issues.
• Carry out assessments, monitoring, and intricate coordination of care.
• Ensure high-quality support while adhering to waiver requirements.
• Facilitate integrated care management through collaboration among multidisciplinary teams and referrals.
• Utilize person-centered planning methods and strategies to collect information and address individual needs effectively.
• Educate and empower members to understand and exercise their rights and responsibilities.
• Complete Discovery activities, which include information gathering and assessments.
• Inform members and their legally responsible representatives about Medicaid, state-funded services, service options, processes, and available natural and community resources.
• Assist members and their legally responsible representatives in making objective choices regarding service providers.
• Serve individuals of all ages—children, adolescents, and adults—with Intellectual and Developmental Disabilities.
• Travel within the designated catchment area as necessary.
• Must fulfill QP status.
• Fully licensed by the NC governing board overseeing Human Services professions; OR a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four (4) years of full-time experience with the MH/DD/SA population; OR possess a master's degree in a human service field with one year of full-time MH/DD/SA experience; OR hold a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; OR have a bachelor's degree in another field and four years of full-time MH/DD/SA experience.
• Substance abuse professionals must possess applicable supervised experience in alcoholism and drug abuse counseling.
• If serving members with LTSS needs, a minimum of two (2) years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience is required.
• A valid driver's license is mandatory.
• Candidates must reside in North Carolina and within 75 miles of Trillium's Southern Region, including Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, or Scotland counties.
• Ability to travel within the catchment area as required is essential.
• All candidates must successfully pass a drug test as a condition of employment.
• Preferred: Bachelor's degree in human services.
• Preferred: Experience with the Innovation Waiver or IDD population, including writing person-centered plans.
