Care Manager

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver comprehensive care coordination and monitoring for members with primary Intellectual or Developmental Disabilities and potential secondary mental health, substance use, or complex medical/physical health requirements.

• Evaluate, oversee, and coordinate care to guarantee quality support and compliance with waiver standards.

• Facilitate integrated care management through collaboration across multidisciplinary teams and referrals.

• Provide services to children, adolescents, and adults with Intellectual/Developmental Disabilities.

• Employ person-centered planning techniques to collect information and meet individual needs.

• Educate and assist members regarding their rights and responsibilities.

• Conduct Discovery activities, which involve information gathering and assessments.

• Inform members and their legally responsible representatives about Medicaid and state-funded services, available service options, requirements, and community resources.

• Support members and their legally responsible representatives in making informed choices about service providers.

• Travel within the designated catchment area as necessary.

⛳️ Requirements

• Must fulfill QP status.

• Fully licensed by the NC governing board overseeing Human Services professions; OR a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing along with four years of full-time experience with the MH/DD/SA population; OR a master’s degree in a human service field with one year of full-time MH/DD/SA experience; OR a bachelor’s degree in a human service field with two years of full-time MH/DD/SA experience; OR a bachelor’s degree in another discipline with four years of full-time MH/DD/SA experience.

• Alternative qualifications for substance abuse professionals require the specified supervised experience in alcoholism and drug abuse counseling.

• If working with members who have LTSS needs, a minimum of two years of previous LTSS and/or HCBS coordination, care delivery monitoring, and care management experience is required.

• A valid driver's license is mandatory.

• Must reside in North Carolina and be within 75 miles of Trillium's South Central Region, specifically in Pender County.

• Ability to travel within the designated catchment area as needed.

• Preferred: Bachelor's degree in human services.

• Preferred: Experience with the Innovation Waiver or IDD population and proficiency in writing person-centered plans.

• Must successfully pass a drug test as a condition of employment.
