This is a fully remote position, open to applicants in North Carolina.
• Deliver intricate care coordination and oversight for members with primary Intellectual or Developmental Disabilities, along with potential secondary mental health, substance use, or complex medical/physical health requirements.
• Perform assessments, monitoring, and intricate coordination of care.
• Guarantee quality support and compliance with waiver regulations.
• Facilitate integrated care management through collaborative multidisciplinary efforts and referrals.
• Employ person-centered planning techniques and strategies to collect information and address individual requirements.
• Offer education and support concerning members' rights and obligations.
• Conduct Discovery activities, including information collection and assessment.
• Inform members and legally responsible parties about Medicaid and state-funded services, service options, requirements, and available natural and community resources.
• Assist members and their legally responsible parties in objectively selecting service providers.
• Must fulfill QP status.
• Fully licensed by the NC governing board that oversees Human Services professions; OR a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four years of full-time MH/DD/SA experience; OR a master's degree in a human service field with one year of full-time MH/DD/SA experience; OR a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; OR a bachelor's degree in a different field along with four years of full-time MH/DD/SA experience.
• Substance abuse professional alternatives require documented supervised experience in alcoholism and drug abuse counseling as specified.
• For members requiring LTSS, a minimum of two years of prior experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management is necessary.
• Must possess a valid driver's license.
• Must reside in North Carolina and within Trillium's Northern Region: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, and Warren counties.
• Must be able to travel within the catchment area as needed.
• All candidates must successfully pass a drug test as a prerequisite for employment.
• Preferred: bachelor's degree in human services.
• Preferred: experience with the Innovation Waiver or IDD population and proficiency in writing person-centered plans.
