This is a fully remote position, open to applicants in North Carolina.
• Evaluate, plan, implement, coordinate, monitor, and assess healthcare options and services.
• Deliver effective care management services to enrolled populations.
• Conduct thorough medical, biopsychosocial, behavioral, spiritual, and cultural assessments.
• Identify strengths and barriers relating to behavioral, social, cultural, and environmental factors.
• Educate members and families on diagnoses, medications, resources, prevention, and risk factors.
• Establish patient-centered SMART goals and assess the quality and effectiveness of interventions.
• Create, review, implement, and evaluate member care plans in collaboration with members, families, providers, and care teams.
• Utilize trauma-informed care, motivational interviewing, strengths-based, and solution-focused methods.
• Coordinate referrals to community-based services and agencies.
• Direct matters beyond the Care Manager’s expertise to the appropriate clinical team members.
• Collaborate with multidisciplinary teams and community provider agencies to achieve treatment outcomes.
• Ensure coordination of services among members, families, guardians, providers, specialists, and care teams to avoid duplication.
• Maintain member documentation in the Care Management platform.
• Achieve monthly productivity and role expectations.
• Adhere to CCNC policies, privacy and security standards, and HIPAA regulations.
• Participate in departmental and corporate meetings, training, and required events.
• Travel within assigned regions and/or throughout North Carolina.
• Registered Nurse (RN): graduation from an accredited nursing program.
• Registered Nurse (RN): active and unrestricted RN license to practice in North Carolina.
• Registered Nurse (RN): at least 2 years of nursing experience.
• Registered Nurse (RN): CCM certification is preferred; certification must be obtained within one year of eligibility according to CCM requirements.
• Registered Nurse (RN): must meet licensure or educational eligibility criteria set by The Commission for Case Management Certification.
• Social Worker: Master’s degree from an accredited school of social work.
• Social Worker: minimum 2 years of social work experience.
• Social Worker: active NC license as a Licensed Clinical Social Worker (LCSW).
• Social Worker: CCM certification is preferred; certification must be obtained within one year of eligibility according to CCM requirements.
• Social Worker: must meet licensure or educational eligibility criteria set by The Commission for Case Management Certification.
• Access to hospital/data or electronic medical record systems as required.
• Valid driver’s license with current auto liability insurance.
• Proficient computer skills, including office software and internet usage.
• Familiarity with government, private-sector, and community resources.
• Understanding of case management principles and relevant federal and state regulations.
• Strong organizational and time management abilities.
• Capable of critical thinking, effective clinical judgment, independent decision-making, and problem-solving.
• Ability to work both independently and as part of a multidisciplinary team.
• Exceptional oral and written communication skills; bilingualism is preferred.
• Sensitivity to cultural diversity, language barriers, health literacy, and various educational levels.
