This is a fully remote position, open to applicants in United States.
• Design, evaluate, and implement intricate care management strategies primarily focused on mental and behavioral health requirements.
• Assess member needs through phone consultations or in-home visits, and propose or facilitate care and service strategies.
• Engage in outreach through telephonic, digital, home, and various site visits.
• Create and maintain ongoing care plans for members with high acuity.
• Identify and connect with providers, specialists, and community resources for mental health and substance use disorder treatment.
• Coordinate efforts among members, families, caregivers, community resources, and care provider teams.
• Oversee care plans, monitor member status, treatment side effects, complications, and clinical symptoms.
• Facilitate care coordination and ensure timely access to necessary care and services.
• Gather, document, and preserve member information and care management activities to ensure regulatory compliance.
• Educate members and families on procedures, provider guidelines, treatment options, referrals, and healthcare benefits.
• Provide feedback on leadership opportunities to enhance care quality and cost-effectiveness.
• Adhere to all policies and standards.
• Perform additional duties as required.
• Candidates must be residents of Georgia.
• An active RN license or a Licensed Master’s Behavioral Health Professional credential is necessary, such as LCSW, LMSW, LMFT, LMHC, or LPC.
• A Master’s degree in Behavioral Health or Social Work, or a degree from an accredited nursing program is required.
• 2–4 years of relevant experience is essential.
• Behavioral health experience is needed for the applicable RN or Licensed Master’s Behavioral Health Professional credential.
• Strong skills in time management and assessment are required.
• Experience with telephonic communication is necessary.
• Excellent communication and customer service skills are a must.
• A solid understanding of behavioral health is required.
• Strong organizational skills are essential.
• Proficiency in computer usage is necessary.
