This is a fully remote position, open to applicants in Kansas.
• Design, evaluate, and implement intricate care management initiatives for individuals primarily facing mental and behavioral health challenges.
• Create tailored care strategies and provide educational resources for members and their families concerning mental health and substance use disorders.
• Assess member requirements through phone consultations or home visits, and suggest or facilitate care/service plans.
• Engage in outreach via telephone, digital platforms, home visits, and other onsite interactions.
• Formulate ongoing care plans for high-acuity members while identifying relevant providers, specialists, and community resources.
• Coordinate efforts among members, families/caregivers, community resources, and care provider teams.
• Oversee care plans, member status, outcomes, treatment side effects, complications, and clinical symptoms.
• Assist in care coordination and work in tandem with providers and specialists to guarantee prompt access to care.
• Gather, document, and uphold member information and care management activities for compliance with regulations.
• Educate members and families on procedures, provider guidelines, treatment alternatives, referrals, and healthcare benefits.
• Offer leadership insights on opportunities to enhance cost-effective care and quality service delivery.
• Adhere to all policies and standards while fulfilling additional duties as assigned.
• Candidates must reside in Kansas.
• Possession of an active Licensed Master’s Behavioral Health Professional license (e.g., LCSW, LMSW, LMFT, LMHC, or LPC) or qualifications that satisfy RN prerequisites with behavioral health experience.
• A Master's degree in Behavioral Health or Social Work, or a qualification from an Accredited School of Nursing.
• 2–4 years of pertinent experience.
• Behavioral health experience is mandatory for RN applicants in accordance with state contract stipulations.
• Strong foundational knowledge of behavioral health clinical practices and assessment capabilities.
• Exceptional communication and customer service skills.
• Preferred experience includes discharge planning, problem-solving, care coordination, proficiency with computer systems, organizational management, time management, and working within multidisciplinary teams.
• Work schedule is Monday through Friday, from 8:00 AM to 5:00 PM.
