This is a fully remote position, open to applicants in Kansas.
• Design, evaluate, and implement intricate care management strategies primarily addressing mental and behavioral health requirements.
• Offer customized care plans and educational resources for members and their families concerning mental health and substance use disorders.
• Assess member needs through phone consultations or in-home visits and suggest or facilitate care/service plans.
• Conduct outreach via telephonic, digital, home, and/or other visits to evaluate member requirements and collaborate with available resources.
• Create ongoing care plans for members experiencing high acuity.
• Identify necessary providers, specialists, and community resources to support care.
• Coordinate communication among members, families/caregivers, community resources, and care provider teams.
• Monitor care plans, member status, outcomes, treatment side effects, complications, and clinical symptoms.
• Facilitate care coordination and partner with providers or specialists to ensure timely access to care.
• Collect, document, and preserve member information and care management activities to ensure regulatory compliance.
• Educate members and families regarding procedures, provider instructions, treatment options, referrals, and healthcare benefits.
• Provide leadership feedback on opportunities to enhance care quality and cost-effective delivery.
• Perform additional tasks as assigned.
• Candidates must live in Reno, McPherson, Rice, or Harvey County to be eligible.
• Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing school.
• 2–4 years of relevant experience.
• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN as per state contract requirements.
• Behavioral health experience is required for RN candidates.
• Experience with foster care populations or within child welfare systems is preferred.
