This is a fully remote position, open to applicants in Kansas.
• Design, evaluate, and implement intricate care management initiatives for individuals primarily facing mental and behavioral health challenges.
• Develop tailored care plans and provide education to members and their families regarding mental health and substance use disorder requirements.
• Assess member needs through phone consultations or in-home visits and suggest or facilitate appropriate care and service plans.
• Engage in outreach through telephonic, digital, home visits, and other site interactions.
• Formulate continuous care plans for members with high acuity levels.
• Identify and connect with providers, specialists, and community resources addressing mental health and substance use disorders.
• Coordinate efforts among members, families, caregivers, community resources, and care provider teams.
• Oversee care plans, member conditions, treatment side effects, complications, and clinical symptoms.
• Facilitate care coordination and ensure timely access to necessary services.
• Record member information and care management actions.
• Educate members and families on procedures, provider guidelines, treatment options, referrals, and healthcare benefits.
• Offer leadership insights on opportunities to enhance quality and cost-effective care delivery.
• Execute other assigned responsibilities as needed.
• Adhere to all relevant policies and standards.
• Candidates must be residents of Kansas.
• A valid Licensed Master’s Behavioral Health Professional license (e.g., LCSW, LMSW, LMFT, LMHC, or LPC) or RN qualifications with behavioral health expertise is required.
• A Master’s degree in Behavioral Health or Social Work, or a degree from an accredited nursing program.
• 2–4 years of pertinent experience.
• Strong clinical knowledge of behavioral health and assessment capabilities.
• Preferred experience in discharge planning, problem-solving, care coordination, computer systems, organizational management, time management, and working with multidisciplinary teams.
• Availability to work Tuesday through Saturday, 10:00 AM to 6:00 PM, or Sunday through Thursday, 10:00 AM to 6:00 PM.
• Compliance with applicable policies and standards is mandatory.
• Required to collect, document, and maintain member information in accordance with state, federal, and third-party payer regulations.
