This is a fully remote position, open to applicants in United States.
• Deliver a comprehensive, client-focused approach to client care.
• Perform assessments and ongoing reassessments.
• Oversee the management of extensive care related to medical and cognitive aspects of aging.
• Formulate and execute detailed care plans.
• Monitor, evaluate, and document changes in client conditions, preferences, challenges/opportunities, medical orders, family requests, and other relevant care plan details.
• Observe, assess, and report on client status, interventions, and outcomes.
• Collaborate with formal and informal social support networks and families to address client needs.
• Offer guidance, resources, and information to assist families and clients in making informed medical choices.
• Manage clients' electronic health records, including billing processes.
• Organize resources for client care.
• Communicate with physicians, attorneys, fiduciaries, hospice staff, and community personnel on behalf of clients.
• Establish and maintain referral resources.
• Identify, coordinate, manage, and advocate for client services.
• Carry out additional responsibilities as assigned by the supervisor.
• Provide assessment and monitoring, planning and problem-solving, education and advocacy, as well as coaching for family caregivers.
• Minimum of 5 years’ experience in community, hospital, hospice, or home health case management.
• Preferred Bachelor’s or master’s degree in a healthcare-related discipline.
• Certification as a Care Manager (CMC) or Certified Case Manager (CCM).
• Proven knowledge of aging, dementia, and/or disabilities.
• Experience with complex family systems and dynamics.
• Proficiency in computer applications: MS Office, Electronic Health Records, and Internet navigation.
• Strong customer service and problem-solving abilities.
• Excellent verbal and written communication skills.
• Detail-oriented with the capacity to manage fast-paced processes.
• Ability to lift and carry weights up to 10–15 pounds.
• Capability to sit, stand, and walk for extended periods during the workday.
• Regular local travel using personal vehicle to client residences is required.
• Proficient in using standard office equipment such as personal computers, laptops, telephones, and photocopiers.
• Applicants from California must provide timely and accurate positive fingerprint identification as a condition of employment.
