Care Management Specialist II

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver professional-level care coordination services within a managed care setting.

• Independently perform thorough assessments of member needs utilizing organizational platforms and available data sources.

• Offer short-term care coordination services tailored to member needs, benefits, services, and community resources.

• Create, implement, and oversee personalized care plans in collaboration with members and support teams.

• Conduct record reviews to identify gaps in care and assist in care planning and service coordination.

• Coordinate services and resources among providers, agencies, and community organizations.

• Facilitate member transitions to guarantee continuity of care.

• Use judgment to align member needs with suitable resources.

⛳️ Requirements

• High school diploma or GED with six (6) years of experience in working with mental health, substance use, and/or intellectual/developmental disability populations; OR Associate's degree with four (4) years of experience; OR Bachelor's degree with two (2) years of experience; OR an equivalent combination of education and experience.

• Valid driver's license is required.

• Must reside in North Carolina.

• Ability to travel within the designated catchment area as needed.

• Preferred: Degree in Human Services.

• Preferred: Bachelor's degree.
