Care Management Specialist

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Provide essential support to Care Managers in delivering suitable services to members.

• Conduct the majority of Tailored Care Management member-required communications.

• Perform outreach, follow-up, and engagement activities with members.

• Meet members either virtually or face-to-face.

• Document health updates of members.

• Recommend adjustments to Care Managers regarding the effectiveness of current care plans.

• Assist members in locating additional medical or community resources.

• Facilitate programs and activities that promote holistic health and enhance members’ health literacy and education.

• Teach self-management techniques and provide self-help recovery resources.

• Distribute educational materials concerning chronic conditions.

• Organize and schedule member services, appointments, wellness reminders, and non-emergency medical transportation.

• Collaborate with Care Managers and other care team members regarding members’ care, health updates, and modifications.

• Participate in relevant educational case conferences.

• Ensure the secure storage and transmission of sensitive health records.

• Work remotely from 8:30 a.m. to 5:00 p.m. and travel within the community as necessary.

⛳️ Requirements

• High school diploma or GED, along with one (1) year of qualifying experience through one of the specified alternative paths.

• One (1) year of lived experience with an intellectual/developmental disability (I/DD) or traumatic brain injury (TBI), exhibiting knowledge of and personal experience navigating the North Carolina Medicaid delivery system; OR

• One (1) year of lived experience with a behavioral health condition and certification as a Peer Support Specialist; OR

• One (1) year of direct experience as a parent or guardian providing care for an individual with an I/DD, TBI, or behavioral health condition while navigating the Medicaid delivery system; OR

• One (1) year of paid experience performing the essential duties described, which includes at least one (1) year working directly with the Tailored Care Management eligible population.

• Valid driver’s license.

• Certification as a Peer Support Specialist if utilized as a qualifying pathway.

• Must reside within Trillium’s Catchment area in North Carolina.

• Must be able to travel within the catchment as required.

• All candidates are required to pass a drug test as a condition of employment.

• Resume must detail employer names, dates of service, average weekly hours, essential job duties, educational background, and relevant licensure/certification information.
