Care Management Assistant

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee administrative, operational, and customer support functions for care management programs and healthcare services.

• Execute advanced administrative, operational, and customer support tasks independently, utilizing sound judgment.

• Assist in assessing, coordinating, and facilitating the care needs of members.

• Work collaboratively with members, providers, caregivers, and care teams to enhance member wellbeing.

• Organize care and review as well as maintain patient records.

• Generate care summaries that inform care teams, direct subsequent steps, and enhance patient outcomes.

• Prioritize requests, interpret and modify procedures, and implement established techniques.

• Report directly to the Associate Director of Consumer Experience.

⛳️ Requirements

• High School Diploma or GED is required.

• Experience in providing in-center or virtual support or engaging with patients.

• Background in patient support, care coordination, or a similar healthcare environment.

• Proficient in data entry and maintaining information within an electronic medical record (EMR) system.

• Ability to work in EST or CST time zones.

• Self-sourced internet service with a minimum download speed of 25 Mbps and an upload speed of 10 Mbps.

• A wireless, wired cable, or DSL connection is recommended.

• Capacity to work from a dedicated space without ongoing interruptions to safeguard member PHI / HIPAA information.

• Fluency in Spanish is preferred.
