This is a fully remote position, open to applicants in United States.

📋 Description

• Offer non-clinical administrative assistance to the care management team.

• Manage incoming and outgoing calls to support care management services.

• Oversee call queues, tasks, and referrals efficiently.

• Collect information via phone to assess eligibility for care management enrollment.

• Direct referrals to the appropriate clinical or nursing teams.

• Conduct outreach to members through phone calls or home visits.

• Discuss care plans and service plans, outlining next steps, resources, questions, and concerns with members.

• Coordinate care activities among healthcare and community providers, partners, members, and caregivers.

• Assist with inquiries from members and providers regarding care plans and service plans.

• Communicate with care managers, practitioners, and others to enhance member services and ensure continuity of care.

• Support service assessments and screenings while documenting member care needs.

• Maintain and document member records, distributing them to providers as necessary.

• Educate members on available benefits and resources.

• Perform additional duties as assigned, adhering to policies and standards.


⛳️ Requirements

• High School diploma or GED is required.

• 1–2 years of relevant experience is preferred.

• Capable of managing inbound and outbound calls effectively.

• Proficient in handling call queues, tasks, and referrals promptly.

• Skilled in gathering information via phone to evaluate eligibility for care management enrollment.

• Able to route referrals to the appropriate clinical or nursing teams.

• Experience in performing member outreach through phone calls or home visits.

• Competent in documenting and maintaining member records in compliance with state and regulatory standards.

• Knowledgeable in following contractual requirements, regulatory guidelines, and standards.

• Ability to identify needs and refer to Care Managers, community-based organizations, and Disease Managers.

• Capable of providing education regarding benefits and available resources.
