This is a fully remote position, open to applicants in Texas.
• Assist in care management activities and designated member teams to guarantee the delivery of provider and partner services, ensuring continuity of care and member satisfaction.
• Perform member outreach through phone calls or home visits.
• Record care/service plans, activities, member requirements, and ongoing educational efforts.
• Engage with members to review care plans, outline next steps, provide resources, address questions and concerns, and recommend care options.
• Coordinate care activities with healthcare and community providers, partners, members, and caregivers.
• Address member and provider inquiries, requests, and concerns related to care/service plans.
• Collaborate with care managers, practitioners, and others to facilitate member services and ensure continuity of care.
• Assist in service assessments and screenings for members.
• Maintain member records in compliance with state and regulatory requirements and distribute them to providers as necessary.
• Educate members on benefits and available resources.
• Execute other assigned responsibilities and adhere to policies and standards.
• High School diploma or GED.
• 1–2 years of relevant experience.
• For the Arkansas Total Care plan: Bachelor's degree in social science or a health-related field, or a high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients.
• For Arkansas: Valid driver's license.
• For Arkansas: Completion of a child and adult maltreatment check prior to hiring and on a recurring basis.
• For Arkansas: Drug screening at the time of hire and on a recurring basis.
• For Arkansas: Must reside in Arkansas or a bordering city.
• For Arkansas: 30% travel is required.
• Capability to identify needs and refer to Care Managers, community-based organizations, and Disease Managers.
• Ability to provide education on benefits and available resources.
