Care Coordinator, Health Navigation

This is a fully remote position, open to applicants in Canada.

📋 Description

• Conduct introductory calls with current patients who are interested in advocacy services.

• Assist patients through the advocacy onboarding process.

• Collaborate with the advocacy team to ensure seamless transitions and proper follow-up.

• Oversee patient communications, including follow-ups, missed calls, and reassignments.

• Keep precise records within Google Workspace, EHR platforms like Healthie, and CRMs such as HubSpot.

• Must be located in Canada.

• Must be comfortable working during Eastern Time Zone hours.

• Must possess empathetic and effective communication skills.

• Must be highly organized and detail-oriented.

• Must be willing to learn and navigate Google Workspace, EHR systems, and CRMs.

• Must align with the mission of enhancing care for older adults.

• Experience in healthcare, customer service, or administrative support is beneficial but not mandatory.
