This is a fully remote position, open to applicants in Illinois.
• Evaluate the care needs of pregnant and postpartum Medicaid members.
• Ensure that members receive suitable services in the least restrictive environment.
• Identify and address obstacles to perinatal and postpartum care.
• Develop and implement interventions to fulfill care requirements.
• Coordinate services and assess case management plans against members' personal objectives.
• Continuously track member progress through assessments, data analysis, conversations, and proactive care planning.
• Conduct assessments and evaluations both via telephone and in person.
• Address immediate and acute gaps in care and access.
• Clarify MCO requirements, services, and benefits to enrollees.
• Provide referrals to community resources.
• Assist members and their families in navigating appropriate care resources.
• Facilitate communication with payers, providers, interdisciplinary teams, and other individuals involved in care.
• Visit Medicaid members in their homes.
• Travel locally up to 25% for collaboration, face-to-face meetings, and field interactions.
• Collaborate with nurses, social workers, behavioral health professionals, and care coordinators to enhance health outcomes and tackle care barriers.
• Certified Doula under Illinois Medicaid.
• Must be a resident of Illinois.
• At least 1 year of experience in the healthcare field.
• Minimum of 1 year of experience in case management or care coordination.
• Intermediate to advanced computer proficiency.
• Familiarity with Microsoft Word, Excel, and Outlook.
• Experience with electronic information applications/software, including electronic medical records.
• Valid driver's license from the state.
• Personal vehicle liability insurance that meets state minimum requirements or specified Humana standards.
• Must undergo TB screening if selected.
• Must have internet service with a minimum of 25 Mbps download and 10 Mbps upload speeds.
• Ability to work from a designated space without interruptions to safeguard member PHI/HIPAA information.
• Willingness to primarily work Monday to Friday, from 8:00 AM to 5:00 PM CST, with flexibility based on business needs.
