Care Coordinator

This is a fully remote position, open to applicants in Alabama, +21 more states.

📋 Description

• Oversee a complete patient caseload via app chat, ensuring messages are clear, warm, and professional.

• Take ownership of response times and quality metrics, consistently achieving service level agreements (SLAs).

• Anticipate patient inquiries and resolve issues with minimal touchpoints.

• Collaborate with pharmacies and laboratories by phone to address prescription concerns, insurance issues, and delivery complications.

• Engage with dermatology providers as necessary to ensure care plans remain on track.

• Keep precise patient records.

• Track patient progress through ongoing messaging, address challenges, and celebrate achievements.

• Provide insights on workflows and patient trends to enhance care delivery.

• Ensure patient responses are completed within 4 hours during scheduled hours.

⛳️ Requirements

• Minimum of 2 years of experience as a chat-based Care Coordinator, care specialist, or a similar position (written chat support, not phone-based).

• Ability to thrive in a fast-paced, metric-driven environment with defined SLAs.

• Comfortable with uncertainty and changing priorities.

• Outstanding written communication skills: ensuring clarity, warmth, and professionalism in every interaction.

• Typing speed of at least 40 words per minute.

• Availability to work full-time from day one, 40 hours per week, 5 days a week within a 9:00 AM to 7:00 PM ET timeframe.

• Must have a laptop or desktop computer (Mac or PC); phones, Chromebooks, and iPads are not supported.

• Reliable internet connection and a private workspace; HIPAA compliance is mandatory.

• Ability to work autonomously and accept constructive feedback.

• High school diploma or equivalent required.

• Candidates must be authorized to work for any employer in the US.

• Candidates must reside in AL, AZ, CT, DC, FL, GA, ID, LA, MD, MI, MO, MS, NJ, NY, NC, OH, PA, SC, TX, UT, VA, WA, or WI.

• Preferred: experience in healthcare or a digital health startup.

• Preferred: background in healthcare or clinical support, including knowledge of pharmacies, labs, or insurance.

• Preferred: familiarity with Slack, Zoom, and Google Workspace.
