This is a fully remote position, open to applicants in Washington.
• Facilitate patient care by ensuring access to suitable resources and programs, and assisting with referrals and provider appointments.
• Conduct outbound calls and address inbound inquiries from patients, providers, and Village resources.
• Perform non-clinical screenings and pathways under the guidance of the Manager; escalate issues to the Integrated Care Nurse as necessary.
• Support the clinical team in advancing patients through the continuum of care by executing assigned tasks.
• Identify qualifying resources and work in collaboration with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to enroll members in programs.
• Communicate directly with patients in a prompt and professional manner.
• Ensure accurate, complete, and confidential documentation is maintained.
• Exhibit outstanding two-way oral and written communication skills.
• Operate in alignment with DaVita’s mission and values to provide high-quality patient care.
• Collaborate closely with Nurse Practitioners and RN Case Managers to coordinate care and facilitate resources.
• High school diploma or GED is mandatory.
• A minimum of 2 years of experience in a professional office setting.
• Capability to manage confidential information with discretion and sound judgment.
• Strong verbal communication abilities are essential.
• Proficient in Microsoft Office, Excel, and Outlook.
• Previous experience with Electronic Medical Records is preferred.
• A commitment to service excellence and patient satisfaction is crucial.
• Ability to work independently while also collaborating effectively with the team.
• A quiet, private home workspace equipped with reliable high-speed internet is required.
• Preferred: 1–3 years of experience in a clinical medical office.
• Experience as a CNA or MA is advantageous.
• Familiarity with care coordination, including referrals and care management.
• Knowledge of navigating insurance payers.
• Experience in direct patient care or support is beneficial.
• Prior experience in a medical office environment is required.
