Care Coordinator

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Ensure the safety of members in both home and community environments.

• Travel to members’ residences for assessments and visits.

• Manage requests and expectations from both internal and external stakeholders.

• Collaborate with members and their families to determine support needs and direct services for health, employment, and independent or community living objectives.

• Work alongside clinical and social support teams to address physical, behavioral health, and long-term service requirements.

• Conduct comprehensive, unbiased face-to-face visits to evaluate members’ physical, behavioral, functional, psychosocial, financial, employment, and independent living needs.

• Utilize established criteria to authorize suitable home- and community-based services.

• Ensure that authorized services are delivered and that members’ needs are effectively met.

⛳️ Requirements

• A minimum of 3 years of experience in a clinical environment.

• At least 2 years of direct clinical experience is mandatory.

• Must be a registered nurse with a valid license in Tennessee, or hold a license in your home state if participating in the Nurse Licensure Compact Act; alternatively, a Master's in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW) is acceptable.

• Outstanding customer service abilities are essential.

• Residency in Warren or Cannon County, Tennessee is required.

• Availability for an 8:00am–5:00pm EST work schedule is necessary.

• Possession of a valid driver's license is required.

• A TB Skin Test is required for coordinators working in field settings.

• Basic proficiency in PC usage, Microsoft Office, and email is expected.

• Strong time management skills are essential.

• Excellent oral and written communication abilities are required.

• Strong interpersonal and organizational skills are needed.

• Various immunizations and/or related medical tests may be necessary.

• A digital literacy assessment is required.

• Employees who regularly operate a company, personal, or rental vehicle must participate in the BCBST Driver Safety Program, possess a valid driver's license, maintain compliant auto insurance, and uphold an acceptable motor vehicle record.
